Forms are available on the website now. It took longer than expected. Please do not submit any till Thursday. Also keep a copy for your records.If filling out the religious exemption, DO NOT fill out section III for religious leader info. It is not needed. The city is trying to get you to supply it. Your belief is YOUR belief.
